Care inquiry information
The care inquiry may collect information such as:

- The expected care schedule
The initial inquiry is designed to collect only the information needed to begin discussing a private care request.
Information not requested through the public inquiry
Do not submit sensitive information that is not required for the initial inquiry.

- Social Insurance Numbers

- Detailed medical records
